Care Coordinator
Umpqua Health is recruiting a Care Coordinator to join our integrated care team and support our mission of improving health outcomes for our members. In this role, you’ll work directly with members to assess their needs, develop individualized care plans, and coordinate essential services that promote wellness, independence, and quality of life. You’ll serve as an advocate and educator, helping members navigate complex health systems and empowering them to manage chronic conditions and preventive care. This position plays a vital role in bridging gaps in care, enhancing access to services, and fostering long-term health and resilience in our communities.
Your Impact:
- Conduct comprehensive assessments in home, telehealth, and community-based settings to identify medical, behavioral, and social needs.
- Develop and implement person-centered care plans that align with members’ health goals, preferences, and support systems.
- Advocate for members by facilitating timely access to medical services, behavioral health care, and community-based resources.
- Provide referrals to specialists, diagnostics, housing services, transportation assistance, and other essential supports.
- Educate members on chronic disease management, medication adherence, lifestyle changes, and preventive care strategies using evidence-based approaches.
- Collaborate with interdisciplinary healthcare teams to coordinate care delivery across providers, facilities, and support systems.
- Monitor member progress, adjust care plans as necessary, and ensure services remain responsive to changing needs.
- Support care transitions between healthcare settings (e.g., hospital to home), ensuring continuity and minimizing gaps in treatment.
- Maintain accurate, timely, and compliant documentation in electronic health records and care coordination systems.
- Participate in health education initiatives, outreach events, and population health programs to advance health equity and literacy.
- Assist members with understanding insurance coverage, scheduling services, and overcoming barriers to care.
- Perform other duties as assigned to support Umpqua Health’s Vision, Mission, and Organizational Values.
Your Credentials:
- Current, active licensure or certification in one of the following: Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Occupational Therapist (OT), Respiratory Therapist (RT), Certified Rehabilitation Counselor (CRC), Certified Disability Management Specialist (CDMS), or equivalent.
- Bachelor’s degree in nursing or related healthcare field preferred.
- Minimum of three (3) to five (5) years of experience in care coordination, case management, or community-based health services.
- Demonstrated ability to assess health conditions, identify care needs, and implement individualized care plans.
- Strong clinical judgment, critical thinking, and problem-solving skills.
- Exceptional interpersonal and communication abilities for building trust with members, families, and healthcare teams.
- Proficient in using electronic health records (EHRs), care management software, and Microsoft Office Suite.
- Ability to manage competing priorities and work effectively in fast-paced, dynamic environments.
- Experience working with diverse populations and a commitment to culturally responsive care.
- Must be in good standing with all applicable licensing or certification boards and eligible to participate in Medicare and Medicaid programs.
